Pain Management Care PC accepts Medicare, Medicaid, and most insurance plans. We are networked with Advantage, Blue Cross & Blue Shield, CareMark, CHA, Cigna, Select Health Network, M-Plan, Principle, Sagamore, and United Health. Individuals with any questions concerning an insurance plan, or our billing process should contact us directly. We will do our utmost to alleviate any financial questions or concerns you might have.
To expedite the scheduling process, we request that prospective patients have a referral from their primary care physician. This is necessary so patient history forms can be quickly ordered, which include medication history reports, radiological reports, and surgical history reports.
New patients should arrive 15 minutes early to their initial appointment in order to complete medical registration forms. We understand that unexpected events do occur that may conflict with scheduled appointments. However, we ask new and existing patients to cancel all appointments 24 hours in advance or as soon as it is possible. Patients who miss a second consecutive appointment without notifying our offices will be billed accordingly.
All prescribed medications currently being administered must be
presented to Dr. Glazier during the first office appointment. In addition,
we ask patients who are currently taking over-the-counter pain relievers
to disclose the drugs name, dosage, and frequency of use.
Finally, new patients should expect an initial medical consultation to create an individualized plan of care. Most plans include a combination of minimally invasive steroid injections, medications, physical therapy, and massage therapy. |
